Osseodensification and Injectable Tissue Engineering: Emerging Frontiers in Implant Dentistry for Compromised Bone

This comprehensive review examines two transformative techniques in contemporary implant dentistry: osseodensification (OD) for biomechanical optimization of implant sites in low-density (D3/D4) bone, and injectable tissue engineering using platelet-rich fibrin (PRF) and mesenchymal stem cells (MSCs) for biological enhancement of bone regeneration and healing. Histomorphometric evidence demonstrates that OD significantly increases bone-to-implant contact (BIC) rates and implant stability quotient (ISQ) values in D3/D4 bone compared to conventional drilling. Injectable PRF, prepared via the low-speed centrifugation concept (LSCC) at 60 × g, delivers a higher-yield growth factor payload in a liquid, syringe-deliverable format that gels in situ. MSC-PRF injectable composites demonstrate synergistic osteogenic activity in vitro and promising phase I/II clinical outcomes in previously ineligible patients. The combined application of both techniques achieves superior primary stability metrics and healing trajectories in D3/D4 bone, with implant survival rates exceeding 96% at two years in published case series. This review provides clinicians with a detailed, evidence-based framework for integrating both modalities in challenging posterior maxillary and low-density bone implant cases.
